When you come for your appointment please remember to obtain and bring the following

• Medicare card or Veteran Affairs card
• Referral letter from your family doctor
• Private insurance details

To cancel an Appointment

Please call our practice during office hours and let us know at least 24 hours in advance so that we can give your allocated time slot to another patient.
